MEDICAL INSURANCE

You will be eligible to partake in our medical and life insurance plans on the first of the month after 60 days. If one does not participate when eligible, then one has to wait for open enrollment, which tends to be late spring of every year. Our plan includes vision and dental coverage. The company pays a large portion of the insurance; the employee pays the difference, which is deducted weekly on a pretax basis four weeks prior to becoming effective. The employee’s weekly contribution for medical depending on plan chosen as follows:

LIFE INSURANCE

Life insurance is given to all employees at no expense to the employee. This benefit equates to 60% of an employee's annual salary. Eligibility begins the 1st of the month after 60 days.

VACATION

Vacation time starts accruing on the first day of work. Employees can earn up to 10 vacations days per year up to the completion of 7 years, then accrual equates to 15 days per year. After 17 years completed service the accrual equates to 20 days per year.

HOLIDAYS

Sheffield Pharmaceuticals offers 8 paid holidays per year: New Years Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving, the day after Thanksgiving and Christmas Day. Eligibility begins 30 days after start date; one must also work the 8-hour scheduled days before and after the holiday and not be more than 15 minutes late on either of the days to be entitled to the paid holiday.

401K PLAN

One becomes eligible for the 401K Plan after one year of employment. The company matches 30% on the 1st 3%, 20% on the next 2%, and 10% thereafter and an employee is 100% vested upon enrollment. To be eligible, you must have been employed for one year, be 21 years of age and work over 20 hours a week.

ATTENDANCE BONUS PROGRAM

We have an in-house monthly attendance bonus program for full time hourly employees. The incentive program is as follows: If you do not miss more than 2 hours within a bonus period (four or five weeks, depending on the month), you will be eligible to receive: Production support employees- $.81 per hour for the regular straight time worked, $1.215 per hour of overtime worked within the period if the attendance bonus criteria is met. Production departments- (Filling & Compounding) are eligible to receive $1.80 per hour for the regular straight time worked,$2.70 per hour of overtime worked within the period if the attendance bonus criteria is met. You must be actively employed the day it is distributed, which is approximately a week after the bonus period ends.

PERSONAL/SICK DAYS

Sheffield Pharmaceuticals div. of Faria Limited LLC, Inc. will provide paid Personal/Sick days, which are accrued at .0124/hour worked which equates to three days per calendar year, to all eligible employees for periods of temporary absence due to personal commitments, illnesses or injuries. Personal/Sick credits can be used in 4-hour increments.

PROFIT SHARING PLAN

On completion of year-end financials, if profits resulted, the company shares at the choice and discretion of the President, up to 20% of profits, minus taxes of these profits amongst active employees. One must be actively employed on the day profits are distributed as well as worked the year of the profit. The first thirteen weeks are not counted as part of the percentage equation for new employees. Profit share is only based on regular time worked. 2006 Profit Share resulted in all qualified employees receiving what equated to approximately 3.4 weeks of wages.

UNIFORM REIMBURSEMENT PLAN

Sheffield offers $100.00/year clothing and $60.00/year for steel-toed footwear reimbursement for personnel in the following departments: Filling, Compounding, Shipping and Maintenance Departments.

EDUCATION TUITION REIMBURSEMENT

Sheffield encourages employees to further their education in job-related fields and assists by offering tuition reimbursement payable upon completion of the course. Reimbursement is based according to grade.

MISC

Evaluations are given at 3-6 months after hire depending on the position. Payroll is done weekly and issued on Wednesdays. Also, direct deposit is available to the bank of your choice.
